Welcome to the Family Chiropractic Center of Santa Fe.  We’re glad that you are choosing to join our family and experience great chiropractic care.  We have been here in Santa Fe since 1983, so we bring to you years of experience successfully treating your condition and many others.  Feel free to call us with any questions that you may have prior to coming in, as well as during your treatment program.  We are here to serve you, so any question is the right question to ask.  We welcome them.

3 Simple Steps For Completing New Patient Intake Forms

Step 1: Below are the links to the intake forms that we will need you to fill out prior to coming in for your first appointment.  If you are paying in cash and are not using any insurance, click on the Cash Patient link.  If you will be using a health insurance, such as Blue Cross or Medicare, click on the Insurance Patient link.  If you have been in a car accident or a work injury, click on the Personal Injury Patient link.

Step 2: DON’T FILL OUT THE FORM FIRST.  Download and save the blank New Patient PDFs to your computer by right clicking your mouse on the link and click “Save link as” or “Save target as” from the drop down menu that appears.  This will prompt a window to open and enable you to save the blank PDF to your computer.  You can then fill out the forms where you’ve saved them.  Once you’ve filled them out, be sure to save them again before sending them.

Step 3: Once you have completed the new patient intake forms, please email the completed PDF forms directly to spchiro@protonmail.com
